When is PPM used instead of percentage?

For very small concentrations (<0.01%) where percentage is unwieldy. Easier to work with whole numbers.

Does PPM conversion to mg/L always hold?

No, depends on density. For water yes (~1 mg/L = 1 PPM), but other fluids require density correction.
