Wells score estimates pulmonary embolism (PE) probability based on clinical findings. It helps guide diagnostic testing decisions.

Steg-för-steg-guide

  1. 1Assign points for clinical features: leg swelling, HR >100, respiration >20, hemoptysis, signs of DVT, etc.
  2. 2Calculate total score
  3. 3Score 0-1 (low), 2-6 (intermediate), or 7+ (high) PE probability

  • Applying score in patients already hospitalized for other reasons
  • Neglecting clinical context when interpreting score

When should Wells score be used?

For non-hospitalized patients with suspected PE. It combines clinical judgment with objective criteria.

What if Wells score is intermediate?

Order D-dimer testing; negative D-dimer essentially excludes PE at this risk level.
