Hawaii was the first state in the United States to mandate employer-provided health insurance through the Prepaid Health Care Act of 1974. This pioneering law required all employers to provide health insurance to employees working 20 or more hours per week, more than 35 years before the federal Affordable Care Act. As a result, Hawaii consistently has one of the lowest uninsured rates of any state and one of the healthiest populations. The mandatory health insurance, combined with TDI and the 12-bracket income tax, gives Hawaii one of the most comprehensive but also most expensive payroll systems for workers in the country.
