When evaluating hypoxaemia, always determine whether the A-a gradient is normal or elevated first. A normal A-a gradient directs you to extra-pulmonary causes (hypoventilation, low FiO2, altitude) and an elevated gradient confirms an intrinsic pulmonary problem.
