- 1Obtain all seven physiological measurements simultaneously or within a short observation window (ideally within 5 minutes). Use calibrated equipment for SpO₂, blood pressure, and temperature. - 2Assign each parameter its NEWS2 score using the band-based scoring table. - 5Document the NEWS2 score on the patient's observation chart, EPR system, or NEWS2 chart. Escalate according to your hospital's escalation policy (e.g., SBAR communication). Trigger additional investigations guided by the clinical picture: blood tests (FBC, U&E, LFTs, CRP, blood cultures, lactate), ECG, chest X-ray, arterial blood gas, or point-of-care ultrasound depending on the likely cause of deterioration.
